Financial Report Forms
Budget change request
This form is submitted if the grant recipient needs to increase one line item’s amount in the grant budget by more than 10%. Due May 1.
Encumbrance Report
This form must be completed to explain any grant project expenditures between July 1 and August 31 which were obligated by June 30 of the grant year. Due September 15.
Personal Services Contracts
This form must be completed for all personnel who receive grant funds to complete grant activities during the lapse period of July 1 to August 31 of any one grant year. Due June 30.
Quarterly Financial Report
This form is submitted at the conclusion of each fiscal quarter to report expenditures of the grant funds. Due October 15, January 15, April 15, and July 15.
Narrative and Statistical Report Forms
First Quarter Narrative Report
The Legislature is requiring quarterly narrative reports for all grants. This form is submitted to report the first quarter progress towards the program’s goals as submitted in the grant application. Due October 15.
Midterm Narrative Report
This form is submitted at midterm of the grant period to report narrative information on the project’s progress toward outcome achievement. Due January 15.
Third Quarter Narrative Report
The Legislature is requiring quarterly narrative reports for all grants. This form is submitted to report the progress towards the program’s goals for the first three quarters as submitted in the grant application. Due April 15.
Final Narrative Report
This form is submitted at the conclusion of the grant period to report narrative information on the project’s progress toward outcome achievement. Due July 15
Statistical Report
This form is submitted twice a year to report the numerical information describing the project’s progress toward its goals. Due January 15 and July 15.
