Duplicate License
A duplicate license contains the exact same information as the original license. If your license has been lost, stolen or destroyed, you may apply for a duplicate license. There is a fee; however, victims of theft age 60 and older will receive a free duplicate license if they present a police report about their driver's license.
You must visit a Driver Services facility for a duplicate license. You will be required to show acceptable forms of identification.

Corrected License
A corrected license contains a change to the information that was on the original license, such as name or address change. There is a fee.
You must visit a Driver Services facility to turn in your incorrect license and to show acceptable forms of identification that prove the change in information.
Within 10 days of changing your name or address, you are required by law to notify the Secretary of State of the change. You can fill out the Address Change form online, but to receive a new license with correct information, you will have to visit a facility to pay the fee.
CDL holders must notify the Secretary of State's office within 10 days of a change in their name or address and must obtain a corrected CDL within 30 days of any such change.
