In 1961 the General Assembly created the Department of Mental Health to assume all responsibilities formerly vested in the Department of Public Welfare (L. 1961, p. 2666). These duties included the care and treatment of all persons admitted or committed to state institutions under the department's control. Most of these institutions performed services for the mentally ill or retarded. Until the creation in 1963 of the Department of Children and Family Services, the Department of Mental Health also operated state institutions and social programs for children and their families. The Department of Mental Health also is responsible for the establishment of programs to prevent and treat alcoholism and drug abuse and the coordination of community services for the emotionally ill and developmentally disabled.
In 1973 a separate division was established within the Department of Mental Health to provide programs for the developmentally disabled. As a reflection of this additional responsibility the department's name became the Department of Mental Health and Developmental Disabilities (P.A. 78-992, p. 96). Files contain copies of audit and other financial reports of departmental divisions and institutions made by private accounting firms or departmental auditors at the request of the Auditor General. Included are audit reports and cash audits which list each agency's name, date of audit, and narrative comment on the agency's scope of activity, management of funds, collections, accounts receivable, appropriations, allotments, liabilities, contracts, leases, inventories, insurance, surety bonds, tabular statements, schedules,
reconciliations, analyses, and summaries of fiscal operations. Each report usually is accompanied by correspondence among accountants, departmental employees, and the Auditor of Public Accounts concerning audit procedures and recommendations.
Also included are quarterly financial reports filed with the department by the Anna State Hospital, Elgin State Hospital, Galesburg State Research Hospital, Illinois State Pediatric Institute, Illinois State Psychiatric Institute, Institute for Juvenile Research, Jacksonville State Hospital, and Manteno State Hospital on their use of federal research grant funds. Files contain correspondence and material relating to construction and permanent improvement projects at various institutions and facilities with the Department of Mental Health. A file or notebook for each project includes correspondence with the supervising architect, the Division of Architecture and Engineering, the Illinois Building Authority, the Capital Development Board, the Governor, departmental employees, contractors, engineers, and architects concerning the planning, financing, and progress of the project.
Files also include land surveys; site plans and drawings; blueline prints and drawings; cost estimates; contracts; notifications of acceptance of bids; preliminary plans and specifications; specifications for plumbing, heating, electrical, and general work; reports on the status of appropriations for permanent improvements; requests for release of funds; authorizations for release of funds; photographs of the work site, buildings, and facilities; change orders; certificates of substantial completion; and minutes of meetings to plan or review the project. Reports filed with the Claims Service concern motor vehicle accidents involving departmental employees while driving state vehicles. Each report lists employee's name, address, driver's license number, and institution or division employed by; name, address, and driver's license number of other driver; date and place accident occurred; description of events surrounding accident; description and license numbers of vehicles involved; and amount of damage to vehicles. Repair estimates and correspondence concerning repairs occasionally accompany reports.

Legal opinions prepared by the Attorney General for the Department of Public Welfare (1921-1960), later the Department of Mental Health (1961-1975), concern the legality of budget appropriations, pending legislation about mental health, taxation for mental health services, the rights of patients and of relatives of patients, the treatment of mentally ill criminals, the determination of mental illness, damage claims by patients and employees, eligibility for departmental care, the estates of patients, independent charitable institutions, loyalty oaths and employee compensation, departmental responsibility in transporting patients, and public assistance. Opinions show the date, file number, summary opinion, full opinion, and occasionally include letters from the director of the department which requested the opinion. Opinions also may indicate whether litigation was initiated by the Attorney General on behalf of the department.

Case files of developmentally disabled persons transferred to the Winneconna Home to learn to adapt to noninstitutional settings include billing authorizations, program planning forms, patient needs and goals statements, admission and discharge record, notices of transfers, and descriptive information forms showing patient's name, Social Security number, birth date, race, sex, marital status, and current living conditions. Files concern land sales and operational management as they relate to the various institutional facilities operated by the department. Land records include quit claim and warranty deeds, abstracts of title, contracts for the sale and purchase of real estate, bills of sale, transfer agreements, survey maps, and related memoranda and correspondence. Also included are leases of buildings and farmland, certificates of tax exempt property, grants and dedications of right-of-way, easements, notices of termination of leases and contracts, pre-annexation agreements and annexation resolutions, property appraisals, and maps of right-of-way areas, proposed annexation areas, and utility line locations.
